Our goal is to assist you with your student inquiries. The Office of the Registrar processes transcript requests, student requests, and other official documentation. All requests must be submitted in writing by student or alumni.

The Office of the Registrar is available to you between the hours of 8:00 a.m. to 5:00 p.m. Pacific Standard Time (PST). USU has partnered with Parchment and uses Parchment Transcript Ordering Services. Please follow the link to be redirected to the Parchment Services. Please refer to the Tuition and Fees section for costs associated with transcript requests.
Once your transcript request is received in our office, the transcript will be processed within 48-72 hours. Please allow an additional 5-7 business days for delivery by the United States Postal Service if you selected the mail option.
Students may request unofficial transcripts and other appropriate records by completing the Documentation Request Form. Students must complete and sign the form using a black or blue ink pen (a signed request must be sent using your USU student email). Typed or electronic signatures will not be accepted. Students should first contact their lender for a deferment form. Students should submit completed forms with all their personal information before submitting.

The Office of the Registrar will not accept any education or degree verification requests over the phone. Any third party wishing to verify an alumni/student’s education or degree must provide USU with the written consent/authorization form signed by the student by submitting their request through Parchment Services. Please allow 48-72 hours for processing.

Students may change legal names only if a legal document showing the new name/name change documentation is submitted to the Office of the Registrar. Examples of accepted documents include a Driver License, a Passport or a Permanent Resident Card, Marriage Certificate, a Court Order documenting a name change, and/or documents from the Social Security Administration.
Students wishing to change their legal name may do so by completing the Student Name Change Form and submitting appropriate documentation. Please be advised that name changes occur at the end of each session.
Students are responsible for maintaining up-to-date information, e.g., residence, phone number, etc.
Students can update their residence by logging onto the Student Portal. Students who relocate while enrolled in a degree program may be unable to complete their studies if they are moving to a country or state where the University is not currently authorized to offer that particular program. Students should contact their Academic Advisor if they are considering relocating during their course of study or if they need assistance updating their demographic information.
The University requires that all new students obtain a student photo identification card. To obtain a student identification card, students will need to complete the Photo Identification Request Form. Students will need to include full legal name, mailing address and a passport style photo. Students who lose their ID may be required to obtain a replacement and pay the replacement fee. Refer to the Tuition and Fee section of this catalog.
Once your request is received, we will mail your student ID within 7-10 business days. A confirmation email will be sent to your student USU email once your photo ID has been mailed.
All degrees are conferred four (4) weeks after final grades are submitted by faculty. USU has partnered with Parchment to offer a free digital diploma along with a paper copy. Your diploma will be available to you within 90 days after your degree is conferred. Your digital diploma compliments your paper diploma but is faster to access and can be shared on social media. Certificates/Diplomas will be mailed to the address provided on Application for Graduation/Program Completion Form. Submission of this document does not confirm attendance for commencement. Information regarding the commencement ceremony may be found under the Students heading on the United States University website by visiting www.usuniversity.edu.
The name printed on the diploma must be the same as on the student’s University record. If needed, students may submit a Student Name Change Form, prior to submitting their application for graduation/degree audit.
